These are great questions!  We can help you answer them.

HOW TO GET STARTED:

 

Step One: Make an Appointment. Schedule an appointment by calling MDS at 704-554-9222.

 

Step Two: Register. Please download the appropriate registration form below. Fill it out, sign it, and return it to us at least 14 days prior to your appointment along with any required deposit. When you to call to make an appointment, we will set a fee for you. Email the completed documents to mdvs@ministryds.org.

 

Step Three: Fill Out Forms. There are three sets of forms below; Adult, College Student, and High School Student.  Please download, complete, and return all forms for your category. Forms appear below. Each form appears in Word Document and PDF formats. You may type directly onto downloaded Word forms. PDF forms must be completed by hand using a pen. All materials must be returned to MDS to be received at least 14 days prior to your appointment.

 

Step Four: Take Some Tests. After we have received your registration form and deposit, we will email you about the inventories you will taking at computer testing sites that we suggest to you. You will also be given a password code that you must use to access the inventories at the testing site. The pass code is good only for one testing administration. Results will be sent to our office and shared with you when we meet you. Testing must be completed at least 14 days prior to your appointment. Failure to complete the testing in time may result in cancellation of your appointment.

 

Step Five: Arrive on Time. Please arrive on time for your appointment. You may have one or more inventories to complete before you see a counselor.

Ministry Development and Vocational Services is based in Charlotte and is extending our assessment programs to the greater New Jersey, Pennsylvania and New York areas in an effort to more conveniently share our services with our partners.   MDVS, formerly Career and Personal Counseling Services, is accredited by the Ministry Development Council.  We are a non-profit, celebrating over 60 years of serving high school students, candidates for ministry, church professionals and clergy as they discern their vocation in ministry. We partner with Presbyteries in the Mid-Atlantic region, the North and South Carolina Synods of the ELCA, MCC, UUA, Western North Carolina UMC, South Carolina UMC, Quaker, American Baptist Churches, UCC, Moravian and other denominations in carrying out our mission.  In extending our services to the greater NJ, NY and PA region, we have also increased our programs to include pastoral counseling and psychotherapy.  This includes individual therapy, couple therapy, pre-marital counseling, family therapy and group therapy.  Our services are available to all members of the community regardless of race, creed, or age.  As a pastoral counseling center, we are committed to taking seriously the religious or spiritual dimension of life and are equally careful to promote no particular religious persuasion or point of view.
